Page to shareHow do I create and publish a form?
Open Peak Forms and describe in one sentence what the form should achieve, or choose a template. Peak creates questions, logic, scores and outcomes as a draft and explains under “Why this structure?” what it assumed. Check everything in the preview, then “Publish”. The link is then under “Share”.
- Open Peak Forms and describe the goal in one sentence, e.g. “Lead form, prioritize companies with revenue above €3M”.
- In the builder, click questions and edit them on the right; add blocks on the left.
- Under “Outcomes”, set who sees what and what reaches you in the Inbox.
- Try “Preview” or “Test”: tests don't count in the results.
- “Publish” – the link and the embed snippet are under “Share”.

Page to shareHow do logic, scores and outcomes work?
Logic shows or hides blocks and skips pages (“Only show if …” right on the block). You award points per answer or by value ranges, grouped into dimensions such as fit or urgency. Outcomes are checked in order: the first one whose conditions apply wins; the default outcome catches the rest. Every response stores why it got which outcome. Everything is fixed rules, not AI.

Open the response under “Responses” and choose “Delete response (GDPR)”, or select several and delete them together. The response is then gone for good. If a Connect contact was created from it, also delete that contact in Peak Connect.

With a hidden field. It reads a value from the address, e.g. ?utm_source=google, and the person doesn't see it. Under “Analytics” → “Sources”, Peak counts by utm_source anyway, otherwise by referring page, otherwise “Direct”.
- In the builder, add the “Hidden field” block.
- On the right under “URL parameter”, enter the name, e.g. utm_campaign.
- Share the form link with the parameter, e.g. …?utm_campaign=fall.

With an outcome of the type “Book a meeting (Peak Booking)”. Whoever gets this outcome sees a button to your booking page, and the meeting lands in Peak Booking. You need a published page in Peak Booking for this.
- In the builder, open “Outcomes” and create or choose an outcome.
- As the type, choose “Book a meeting (Peak Booking)”.
- Under “Booking page”, choose the page and adjust the button label.
- Under “When does this outcome apply?”, set the condition, e.g. points or an answer.

Frequently asked
4What happens to the live form when I edit it?Answer
Page to shareWhat happens to the live form when I edit it?
Nothing. You always edit a draft; the live form only changes when you publish a new version. Published versions stay unchanged, every response remembers its version, and under “Share” you can put an earlier version live again or take it over as a draft.
Where do responses go, and who sees them?Answer
Page to shareWhere do responses go, and who sees them?
Responses are under “Responses” in Peak Forms, visible to you and – for a company's forms – its owners and admins. Anyone who provides an email becomes your contact in Peak Connect (the same email finds the same contact). Outcomes with “Notify me in my Inbox” appear in the Inbox, and “Follow up as a task” creates a task in Peak Work. The person filling in the form never sees scores or the decision.

2Why can't I publish my form?Error message
Page to shareWhy can't I publish my form?
The quality check found at least one blocking issue: usually a default outcome is missing, a rule refers to a deleted question, a choice has fewer than two answers, or a “Book a meeting” outcome has no booking page. Under “Quality”, each issue is listed with a link to the block.

Page to shareAn error message appears when submitting my form. What does it mean?
“Please check the highlighted answers.” means a required field is missing or a format doesn't fit. If a lot of responses arrive at the same time, Peak briefly slows down. If the form is paused, it doesn't accept anything. In all cases the input stays in the form; nothing was sent.
